    Muscle: the only organ that gives you your calories back

    Losing weight by losing muscle is guaranteeing the regain. Muscle is the most metabolically active organ in your body: protecting it during weight loss changes everything long-term.

    Chapter 01

    Muscle burns even at rest

    One kilogram of muscle consumes about 13 kcal per day at rest, versus 4 for one kilogram of adipose tissue. Over a year, 5 kg of additional muscle represents nearly 24,000 kcal passively burned — the equivalent of 3 kg of fat.

    This figure looks modest but is constant, daily, effortless. It explains why a muscular person can eat more without gaining, at equal activity.

    The trap of poorly conducted low-calorie diets

    Aggressive caloric restriction, without sufficient protein intake or resistance exercise, causes simultaneous loss of fat and muscle. The typical ratio observed: 25 to 30% of weight loss comes from muscle (Cava et al., Advances in Nutrition, 2023).

    Result: at diet's end, resting metabolism is lower than before. Resuming usual food then triggers regain… essentially as fat. This is the metabolic yo-yo mechanism.

    Chapter 03

    Semaglutide and lean mass loss

    The STEP trials (Wilding et al., NEJM 2021, followed 2022-2024) document that 20 to 40% of weight loss under semaglutide comes from lean mass. Without resistance exercise or high protein intake, the effect is amplified.

    This is a crucial point that prescribers are only beginning to integrate: the weight Ritalin works, but does not exempt from moving and eating enough proteins. Otherwise, regain on discontinuation is almost guaranteed.

    Chapter 04

    Recommended protein intake

    During weight loss, the protective protein threshold is higher than at maintenance: 1.2 to 1.6 g per kilogram of body weight per day, split across 3 to 4 servings (Longland et al., Am J Clin Nutr, 2023). In older adults, this rises to 1.5-2 g/kg/day.

    Sources: eggs, fish, lean meats, tofu, tempeh, pulses (combined with grains for complete profile), skyr and Greek yoghurt, quality protein powder if needed. Tofu provides about 15 g of protein per 100 g, one egg about 6 g.

    Chapter 05

    Resistance exercise: essential

    Two to three muscle-strengthening sessions per week, working the major groups (legs, back, chest, core), suffice to preserve lean mass during weight loss. Gym is not mandatory: resistance bands, bodyweight (squats, push-ups, assisted pull-ups), loaded stair climbing all work.

    Combining resistance and aerobic (brisk walking, cycling, swimming) yields the best body composition results. Walking alone does not preserve muscle in a caloric deficit.
