    Food order: free, immediate, documented

    The same meal, eaten in a different order, does not produce the same glucose peak or insulin response. It is one of the most documented and easiest levers to apply.

    Chapter 01

    What the research measured

    Shukla et al. (BMJ Open Diabetes Research & Care, 2023) had 15 type 2 diabetic volunteers eat exactly the same meal (proteins, vegetables, carbs), but in different orders. Result: the postprandial glucose peak is reduced by 37% when vegetables and proteins are eaten before carbs, versus the reverse order.

    These results were reproduced in non-diabetic individuals (Kubota et al., Nutrients, 2020), with a more modest but significant effect: -20% glucose peak, -25% insulin secretion.

    Chapter 02

    The mechanism

    Fibres and proteins slow gastric emptying. When they arrive in the stomach first, they create a "carpet" that delays carb passage into the duodenum. Glucose is then absorbed more slowly, the glucose rise is spread over time, insulin rises less sharply.

    Second effect: proteins and fibres stimulate GLP-1 and PYY secretion, the two satiety hormones. Eating bread at the end of the meal, when you are already sated, spontaneously reduces the quantity consumed.

    Chapter 03

    Concrete application

    At the restaurant: order a starter of crudités or vegetables, then attack the main by starting with proteins and vegetables before starches.

    At home: serve crudités or salad first, wait 5 to 10 minutes before the main. Within the plate, eat vegetables and proteins first, finish with starches.

    Breakfast: start with Greek yoghurt or eggs, then fruits or cereals. Avoid fruit juice as a fasted opener (maximal glucose peak).

    Chapter 04

    A starter trick: vinegar

    A tablespoon of vinegar (cider, wine, balsamic) in a starter vinaigrette further reduces the glucose peak of the following meal (Johnston et al., J Am Diet Assoc, 2020; Petsiou 2014 meta-analysis). Acetic acid partially inhibits salivary and pancreatic α-amylases, slowing starch digestion.

    The effect is modest (5 to 10% additional peak reduction) but adds to food-order effect. Cost: almost none. Side effects: heartburn in sensitive individuals, to avoid in reflux or ulcer.

    Chapter 05

    Limits of the method

    Food order does not compensate for globally unbalanced eating. A giant dessert at the end of a correctly ordered meal remains problematic.

    It is a complementary lever, particularly powerful in prediabetes or insulin resistance. In metabolically healthy individuals, the effect is more modest but still beneficial long-term.

    Frequently asked questions

    Should I wait between starter and main?
    Ideally 5 to 10 minutes, to give fibres and proteins time to reach the stomach. Not indispensable: even consumed one after another in the right order, the effect remains significant.
    Does the order work for non-diabetics?
    Yes, more modestly. Glucose peak reduction is 15 to 25% in metabolically healthy individuals, versus 30 to 40% in type 2 diabetics.
    Does fruit at meal end cause weight gain?
    No. The myth comes from a supposed "fermentation" with no physiological basis. At meal end, fruit even provokes less glucose peak than when fasting, thanks to the slowing effect of other foods.
    Is vinegar for everyone?
    No. To avoid in gastro-oesophageal reflux, ulcer or gastric sensitivity. In tolerant individuals, a tablespoon diluted in vinaigrette is well accepted.
    Can this principle be applied to a sandwich?
    Hardly as is, everything is mixed. A trick: consume a small starter (crudités, a few almonds, a hard-boiled egg) 5 minutes before the sandwich to prime the satiety signal and slow gastric emptying.
