Free fructose: the friend that wants to harm you
The fructose in a whole apple does not do what the fructose in a soda does. Understanding why lets you escape a major metabolic trap, often invisible in the modern diet.

Two very different fructoses
The fructose naturally present in whole fruit comes with a package: fibres, water, polyphenols, vitamin C, pectins. Absorption is slow, satiety builds, and part of the fructose is captured by intestinal cells.
Free fructose — from sodas, juices, syrups and processed products — reaches the liver unchecked. This form is the problem, not whole fruit.
Direct transit to the liver
Unlike glucose, fructose does not use insulin to enter cells. It is almost entirely captured by the liver (via the GLUT5 transporter), following a metabolic pathway that bypasses normal regulation.
Once in the hepatocyte, fructose is phosphorylated without limit, consumes ATP, and feeds de novo lipogenesis: the creation of fat from scratch. This fat accumulates in the liver (hepatic steatosis), then in the blood as triglycerides.
Non-alcoholic fatty liver disease: silent and common
NAFLD (non-alcoholic fatty liver disease, renamed MASLD in 2023) now affects 25 to 30% of European adults (Journal of Hepatology, 2024). It is asymptomatic for years, then can progress to steatohepatitis, fibrosis, even cirrhosis.
Regular consumption of sweetened beverages is one of the three major risk factors, alongside abdominal overweight and sedentarity. A prospective study of 350,000 adults (JAMA Internal Medicine, 2023) documented a 21% increase in all-cause mortality risk among those consuming more than 2 sweetened drinks per day.
Glucose-fructose syrups
Glucose-fructose syrup (HFCS in the US) is present in most processed products: sauces, biscuits, ready meals, sweetened yoghurts. Its fructose fraction can reach 55 to 65%.
Reading labels is tedious but worthwhile: glucose-fructose syrup, high-fructose corn syrup, agave syrup (85% fructose), concentrated grape juice. All behave the same way at the liver.
What whole fruit brings, by contrast
An apple contains about 10 g of fructose, but also 4 g of fibres, pectin, vitamin C and quercetin. Fibre slows absorption, pectin protects the intestinal lining, polyphenols dampen postprandial inflammation.
Large cohorts (Nurses' Health Study, EPIC) associate regular whole-fruit intake with reduced overweight and diabetes risk, despite the fructose. Form and context make all the difference.

- Should I cut fruit to lose weight?
- No. Whole fruit remains beneficial: its fructose is absorbed slowly, satiety builds, fibres and polyphenols protect the liver. Juices, however, behave like sodas.
- Is honey better than white sugar?
- Marginally. Honey brings some antioxidants and enzymes, but is still 40% free fructose. Used in small amounts, not problematic. In large amounts, it is.
- How many sodas per week are acceptable?
- WHO guidance aims at zero. Occasional consumption (less than one per week) has no significant impact on liver markers in an otherwise healthy adult.
- Is a smoothie equivalent to whole fruit?
- No. Liquid texture speeds gastric emptying and absorption. Blending destroys part of the fibre matrix. A smoothie raises glucose more than the same fruit bitten.
- Can hepatic steatosis be reversed by diet?
- Yes, in early stages. A 7 to 10% body weight reduction, combined with cutting sweetened beverages and a Mediterranean diet, produces documented steatosis regression in 6 to 12 months.
